Registration/Photo-Release Forms

Please fill out the following forms and either email them to Meghan Fox (Meghan.fox@unchealth.unc.edu) or give them in person at the hospital.

• Photo-Release Form
  • This is mainly giving permission for your child and family members to be seen on camera with others during the virtual sessions. You can specifically mark if you want your photos/videos to be shown on social media.
• Registration Form
  • To access the fillable version, you need to download the PDF.
  • To fill out the signature portion, open the PDF in Adobe Acrobat and sign digitally OR print it out and sign manually.
